My Health Record is a secure website where you can access your digital health records shared by our six partner organizations:

  • Brockville General Hospital

  • Lennox & Addington County General Hospital

  • Providence Care

  • Kingston Health Sciences Centre

  • Perth and Smiths Falls District Hospital

  • Quinte Health

We are looking for your input on:

  • the design of the portal and the portal’s mobile app

  • how easy it is to navigate and find information

  • accessibility for devices such as computers, laptops, tablets and mobile phones

  • materials such as handouts, videos, how-to guides and website information

Trusted Person Access

Are you a Trusted Person (caregiver, Substitute Decision Maker) for another person? Please download and fill out the Consent Form and Access Account Form to request access and provide consent from the person whose records you wish to access.

For clients under 12 years old: Access to a client’s health information by a trusted person is automatically removed when a client turns 12 years old.  When the age of 12 is reached, allowing trusted person access is considered optional.    Clients 12 years old and older can choose to remove a trusted person’s access to their health information at any point in time
