My Health Record Portal FAQs

What is My Health Record? 

My Health Record is a secure portal (website) for patients and residents to access their digital health records shared by our six partner organizations: 
  • Brockville General Hospital 
  • Kingston Health Sciences Centre 
  • Lennox & Addington County General Hospital 
  • Perth and Smiths Falls District Hospital 
  • Providence Care 
  • Quinte Health 

When will it be available to others? 

The portal is currently available to a small group of testers, including some Experience /Patient Advisors and some patients in Stroke Prevention and Diabetes Education clinics. We do not yet have a date for a broader launch of the portal. 

What can I see in the portal during the testing period? 

You will be able to: 
  • View and manage their appointments at our partner organizations 
  • Access test results and some notes from their appointments 
  • See historical information from December 2024 onwards 
Our Pilot does not include the following features: 
  • Ability to add your own information (e.g. medications, allergies) 
  • Information / test results from your family doctor / testing facilities outside of the hospital 
  • Self-enrolment (registration is currently limited to a small trial group) 
  • Patient/Physician messaging 
  • Self-scheduling for appointments 
 

What do I do if there’s missing or incorrect information? 

There is a 10-day delay in pathology / lab results and a 14-day delay for diagnostic imaging results, so you may need to check back again to see your test results. Please visit our support page for contact information if you have questions about information in your record. Please note that our medical records teams cannot answer questions about test results or treatments. 

Can I upload my own information, such as external test results, medications or information about my health? 

This feature is not available during the pilot phase, but will be available in the future. 

How do I arrange for others, such as a caregiver, to view my information? 

Access to a portal account by another individual is known as Proxy Access. Proxy Access can be requested by completing a Proxy Request Form to obtain a person’s consent to allow other individuals to access their personal health information through the portal.  To request this, access please the organization where you received care:

How secure is my information? 

The My Health Record Portal has strict privacy and security standards, along with 24/7 monitoring and support. The data is stored in Ontario, and compliant with Ontario laws and regulations. 

 

What do I do if I have trouble logging in, or have technical issues accessing parts of the portal? 

Please contact the 24/7 support line at 1-888-249-0074. This is a toll-free number to the Cerner Customer Care Support Centre, located in Kansas City, Missouri. They can help with log in issues, password resets, navigation questions, or other technical issues you may experience.
